1.) Postpaid subscribers in dtac 1800 system registered as an individual and aged between 15-75 years old (not including juristic persons and government and
corporate organizations) are eligible to apply for dtac lifecare program as details below:
1.1) The subscriber can register to this program during 8 October 2009 - 30 June 2010.
1.2) To be entitled to the insurance benefit coverage for the first time, the eligible subscriber shall make payment in full amount within the due date for 3 consecutive
billing cycles. The insurance benefit coverage will be effective from a payment date of the third billing cycle until a due date of the next billing cycle.
1.3) The insurance coverage period under dtac lifecare program will be covered on a billing cycle that the subscriber makes payment in
full amount within the due
date and shall be extended on a monthly basis when the subscriber makes payment in full amount within
the due payment. The insurance benefit coverage will
be extended from the payment date until the due date in the next billing cycle.
1.4) The subscriber can register via various channels as follows:
- Interactive voice response (IVR): Dial *1402 (free of charge)
- Express mobile service (USSD): Press *142# and dial for SMS reply in Thai and *142*9# and dial for SMS reply in English (free of charge)

1.5) The insurance benefit coverage of dtac lifecare program starts from 1 November 2009 untill 31 October 2010. The insurance benefits are subjected to each
condition above.
2.) In case that there is a request of postpone payment ,the subscriber is still required to make payment in full amount within the due date in order to renew
the insurance benefit coverage.
3.) In case that the subscriber registered to the program more than one number, every number that makes payment in accordance with the above conditions will be
entitled to the sum insurance coverage not more than 1,000,000 Baht.
4.) The subscriber shall be entitled to the insurance benefit coverage when his/her qualification fulfills terms and conditions of the program without having to send any
document or pay any extra fee. If the subscriber transfers the number, changes the number, cancels the service, or change to prepaid system, membership and the
insurance benefit coverage will be terminated.
5.) Conditions for claiming accidental death benefit for the subscriber entitled to the insurance benefit coverage are as follows:
5.1) The coverage of dtac lifecare program applies only to loss of life caused by an accident.
5.2) The insured shall have the name registered as the owner of that number at the time of death and beneficiaries shall be legally an heir.
5.3) To file a claim for death benefit, beneficiaries shall notify the service provider or Muang Thai Life Insurance within six months from the date the insured passed
away.
5.4) The amount of coverage for each number depends on a period of usage of that number. Principally, a reference will be made to data stored in computer system
of the service provider.
5.5) Coverage agreement, exclusions, and disclaimers shall adhere to the insurance policy of Muang Thai Life Insurance. For further information, please contact

6.) The service provider reserves the right to cancel and change any promotional detail where appropriate. In such case, the Subscriber shall be notified when any
